The ability to pull clinical data from participating practices on a regular basis for a relatively small cost allows the organizations and practices within the DI networks to compare their performance on many measures to each other. We do this for specific projects as well as across the DARTNet Collaborative for interested practices. Specifically, the goal is to compile and process EHR data in order to provide participating organizations/practices a graphical view of how they are doing on specific measures and how they compare to other organizations/practices. In addition, the reports seed a sense of community to share best practices.

The current practice performance registry includes 209 primary care clinical sites, just over 1,000 clinicians, and over 1.5M patient records. The system currently tracks clinical performance for diabetes, blood pressure control, lipid control, vaccination for pneumococcal disease, and chronic kidney disease. Preventive services reports are planned. The system allows users to select from over 25 different metrics. While this is a good start, we are working aggressively to move from data to action. This is not as simple as it may seem. The DARTNet Collaborative has explored how to improve this process using multiple approaches and continues to learn and improve the process.
