DARTNet News
Two DARTNet member organizations received funding to support innovation in providing collaborative primary care and mental health care. The Advancing Care Together (ACT) project will gather local, state and national leaders focused on improving primary care, mental health care and substance abuse treatment.
The DARTNet member organizations receiving ACT grants were Miramont Family Medicine in Fort Collins, CO, led by John Bender, MD, and Westminster Medical Clinic in Westminster, CO, led by Scott Hammond, MD.
The three-year ACT program is funded by the Colorado Health Foundation.

See the Members Site for practice performance reports on type 2 diabetes (hemoglobin A1c levels, lipids, microalbumin, and blood pressure), hypertension (blood pressure), chronic kidney disease (glomerular filtration rate), hyperlipidemia, and Pneumovax immunizations.

SAFTINet – a network of safety net organizations such as community health centers
STARNet – primary care PBRN located in South Texas
UNYNet –upstate New York PBRN
WPRN – a network of primary care practices serving Washington, Wyoming, Alaska, Montana, and Idaho
